Looking at some of the categories for the first time and I see many threads are marked as read even tho I've never visited that category before. How do I know what I have and have not read if threads are randomly marked as read?

I think the empty 'comment icons' in the Categories screen just means that no one has posted in that forum yet - rather than you've already been there!
Yes rather confusing, but that's the answer. Communication design in this software needs an overhaul.

Content is considered read by default if it was posted before registration date, or is older than specified threshold which defaults to 40 days AFAIR.
Empty comment icon on light background means that category contains no unread content.
